Thought I'd share a minor snag I had with getting my police certificate. I changed my name several years ago (to match my wife's surname) and when I submitted the subject access form I put down my previous name.
However, when the certificate came back it only had my current name on it. Now, as required I had informed NZIS of my name change, and when they received the certificate they quite rightly pointed out that this didn't confirm I had a clean slate under all names I had been known under.
Initially my county police service data access office weren't very helpful, but after some cajoling they chased up New Scotland Yard for me directly, whi confirmed that if I sent them a letter stating the reference numebr on the first certificate they sent out, and stated what I needed, they would send out a new letter confirming checks had taken place under both names.
Unfortunately, they also stated that certificates as standard do not include all prior names. Be aware you may need to allow a little extra time to send off for the second letter, and you may as well go through that process before submitting you ITA pack. Here is the address you need to send your request to. Remember to include your original certificate reference number, and that you need the letter to state that checks were made under all your names, and to list them.
Dunno if anyone else had this problem, but it may save you a little extra stress.
