I found two "USA immigration physical exam" doctors 10 miles form home who can perform the NZ Health Exam (NZIS 1007) from the following web site :

I went to both of them with my NZIS 1007 form for a "price quote".
The first one read the complete form & quoted me US $85 for the exam only (kid under 15 does not need blood test), US $240 for exam & all standard blood tests (excluding the discretionary tests). Then they refer me to the x-ray center which is an additional US $87. Some insurance might cover the X-ray portion of it.
The second doctor seem to be about US $30 cheaper per person. But they are unfriendly & not willing to put up the effort for us.
I was able to make the appointments of all 3 of us the next day with the first doctor. All tests are done now & waiting for results on Monday (2 business days later). All went well. :clap We also have a very interesing discussion on the BMI issue.
