Nearly there with evidence for NZQA but not sure about this;
'Previously used names' - 'Also include a certified copy of documents showing your change of name eg marriage certificate'

Should I send the origianal or get a photocopy certified? Surely the original is better?

I sent a certified copy as I don't think they actually need to see the original.
As NZQA do courier all your docs back to you eventually I think sending the original would be fine except that I would make it very clear on the covering letter that you have sent the original and not a copy incase!!
