I have sent out my ITA yesterday afternoon after after reading the photocopied forms again, I was wondering if I need to attach the x-ray film taken by the radiologists or do I need to just send the medical and x -ray forms only.
Your help in this matter is greatly appreciated.

you only need to send back the forms and the radiolagists (sp) report, you can keep the x-ray film

Yet another posting to confuse me. I was told by the medical centre who did my medical, that I had to send the x-ray pic aswell as the written report.
I guess it just depends where you go and who you get your info from. I sent the whole lot just to be on the safe side.

I am sure it says on the medical form not to send the X-rays only the second part of the form that has been filled in by the radiolagist (sp).

you keep the x-rays, hang them on your wall and look at them and say i spent 60 quid for that artwork.
Would you pay 60 quid for a picture?

Ah damn it .... looks like I've handed over a unique piece of 60 quid art. I just hope they appreciate it and hang it in a place where it will be seen by all ...... after all, I have a fantastic rib cage !!!!!!

just had xrays done at york district hospital!! receptionist handed over results in v large brown cardboard envelope and said you must not open smaller envelopes with nzis forms until you go for medical in new year!!!! :( but ok to look at film!!?! mentioned to receptionist that we had just paid £146 for privelige of being xrayed and will take a look if we want to, went round corner to a little coffee shop selling flat whites!!!!!!! and took this as a sign :mrgreen: :mrgreen: peeled open smaller envelopes to peek at results and all clear :nice1 :nice1 enjoyed 2 large flat whites + 1 b&h gold , blisss

hi ali
even if you were totally honest and did what the xray people told you (not to open the smaller envelopes), any other doctor or xray technician would have been able to tell you that your xray results were normal. so you did the best thing heehee. ;)
that's good for you. excuse me though, what are flat whites?

A flat white is a cup of coffee. However, if you go into a cafe know and ask for a cup of coffee they almost ask you if you want a bucket of water for your woolie mammoth.
It's all flat white, cafe latte and every other daft name you can think of. The other day I saw an ad on telly telling us about some new coffee which gives a (quote) luxurious cafe creme (pronounced 'cremma') layer on top.......... what's wrong with calling it frothy coffee ???????
Cafe society gone crazy me thinks.

hi annie,
flat whites are a coffee that i've only found in oz/nz, it's like a latte but with no froth and just one shot of expresso! delicious (especially if made with 'illy' brand coffee) extra delish if choc muffin supplied as well :angel
it was just a bit of a surprise to find a coffee shop in uk selling flat whites, after a chat to the owner found out he had lived in auckland for four years ;) and loved them too!!! will be going back for another soon, and will push the boat out and have a 'flattie' with vanilla syrup...mmmmmmmm. (well it is christmas after all) :P

Guys,
I may be wrong but on the form (page 10 to be precise) I received from NZIS, it states the following...
"IMPORTANT
Please do not fold X-ray plates (film). Use a flat envelope and preferably with a thick cardboard backing sheet. On every X-ray plate please write the full name of the applicant, and the date of birth."
I took it to mean that they want us to send the plates to them. Why else would NZIS say use a flat envelope?
We have not sent our ITA yet but will be sending the plates with the ITA.

Always best to be safe, but when I offered the plates to our NZIS case worker she said they only want to see them if something is found on the x-ray. i.e. something not normal
Our doctor advised us to keep them for our records. Ours only cost £35 and in London!!!
