Hi
Just a quick question about the Medical Forms. We are off for our medicals tomorrow. Do we fill in the medical history stuff before we go or does the doctor have to do this when we are there.
Also I am having to print them off as they did not come with the ITA. Is this the same for everyone or is it just us.

You don't need to print them off, the doctor will have a stock of them.
We filled out the medical sections when we got there, whole process only took 30 minutes for both of us.

Ooops just finished printing off my 5th form.
But I think we need to take them to the xray place first, as they are not done in the same place. So even if doc has them, we are probably better having them with us for the xray.

We printed out forms off and took them with us.
Filled the top part in on the xray, and then they filled in the rest. The xray person got a bit narky with us as she said we weren't ment to fill them in beforehand, but to do it infront of her.
Left the medical ones blank and filled it in with the Dr.

We had our forms sent to us when we made the appointment. We filled in all the medical history questions and then went through them with the doc on the day.

We got ours with the ITA.
If you read the first page, you will notice that we need to fill the first part of each section (medical and x-ray) before you see the doctor and teh remainder in his or her presence.

According to the clinic where we will take our medical, we have to answer sect. A and B1 to B4 before going there.
Yes, there was a form in our ITA pack (but the CD was missed out)

Don't worry about it Jinkee. Whatever that is in the CD is available at the NZIS website!
I have a soft copy of the CD too. PM me an email address I will gladly send it off to you! It isn't too big a file once I zip it!
