There will be a form with the ITA (most of which is just a printout of your EOI) telling you what documentation you need to supply - things like birth certificates, originals of qualifications, proof of a relationship, your passports, and of course the medical forms.

Got mine on wednesday - its a great feeling. :raebanana Congratulations.
There is a form and a LOT of advice in the pack to tell you what you need to do and include.
I suggest you try to read the pack a few times, it will then seem less daunting.
