My doctor has made some mistakes.
The first two cost me some time, but I fixed them.
Today I was reviewing my application when I realized the syphilis exam was missing. He hadn't requested that.
I'm also worried about the fact that he removed the pages 13 and 14, saying they weren't necessary.
It's clear that the syphilis exam is compulsory. Does anyone know about not sending the pages 13 and 14? It's the "Laboratory referral form".
