We are having our medicals next week and was just wondering...

- do we take printed off copies of the forms with us

- do we fill in the bits of the form such as name and address

- will my 13 yr old need to provide urine sample... could be tricky that one..

Our Dr has asked us to fill in pages 1-5 but not to sign the declaration until in front of him.
Also to fill in the name and address bits on the X ray pages and Lab report pages.
As far as the urine sample goes i think you have to provide one for anyone over 5!:uhoh Just get your's to drink gallons of water just beforehand and then send to the loo with taps running etc thinking watery thoughts:laugh

Yep, it does say - on the front of the form I think - which sections to fill in beforehand... You can print the form off the NZIS website, someone will be along with a link shortly no doubt... It won't hurt if you miss a bit, but I imagine the doc won't be too pleased if you sit and fill in the whole thing while you're there! Don't sign anything until you get there though.

We had ours done in Jan. We printed the forms off the NZIS website, one set(about 14 pages) for each person. It tells you which bits to fill in advance and which sections will be filled in with the examiner. We had great fun with our 7 year old daughter and the urine. After lots of trips to the toilet, bribary with chocolate and a huge bottle of water she finally went! Good luck with the medicals.

I maybe a bit thick but - do you have to do the urine sample before you go (i.e. one I made earlier) or do they give you the bottles to use when you get there?

Yes, thats right, you do - joy!
Glad out 5yr didnt have to - god knows how long we would have been there and it felt like we were there ages anyway!

I also filled in as much of the paperwork as i could for all tests, clipped the photographs to the paperwork and only signed when at Drs.

Yes, pee at the medical. Our 7 yr old son managed, but would not want a young girl to try.
Fill in personal parts of the form to save time when there.
If you haven't then download the forms (1 each). There are lots of pages, so may be better on your printer to print 1 and then photocopy the rest?

:exit Real dur brain question but photos need to be attatched, do I just use a paper clip or pin to 'exact' spot?

Same with ITA's - do I paperclip to top of page or pin to exact spot.

With these questions I know I'm going dotty :exit

We just took the photos loose and the examiner stapled them on the form at the surgery.
Not sure about the ITA as we are still waiting(forever).

I'd not thought of that. I think I might email my Visa Officer and ask her. She was quick to respond to another question I asked.

They need to be attached next the names but not necessarily in the boxes.
