I'm filling in the form "Leaving the United Kingdom". Two areas I haven't the foggiest is what tax ref. do I put in the top right hand box & how do I find out what tax office to send it to? I haven't been registered for tax for about 7 years & all that stuff was binned before I came over. OH's is filled out ready to post with all the correct info which I took from the accountant's final accounts.

Not 100% certain who to send the form to but you should be able to give the taxman a call and ask them:
http://www.hmrc.gov.uk/cnr/contact_a.htm
With respect to the Tax Ref I think that provided you have your NI number there you don't need to worry about it.

If you know your PAYE reference or office number which you should be able to find on your P45 or recent wage slip then you can use Tax Office Locator (http://search2.hmrc.gov.uk/kbroker/hmrc/locator/locator.jsp?type=1) to find the correct office.
For your tax ref number, I agree with James, if you can't find your number then your NI number will be sufficient.
