Hello to all,

Does anybody know whether we have to submit our blood report within medical and x-ray certificate along with ITA application? Regarding the payment for ITA application, is it only sufficient to mention credit card info on payment detials on ITA form if I am paying VISA card? If not, please mention the process how to make payment for ITA application fee.

Lastly, can somebody know whether we can add additional new pages for Amendments or New information since my amendments won't fit on that single page provided along with ITA.

yes, your medical and x-ray needs to go with your ITA. They will want payment when you submit.

Just to clarify. It is my understanding that the entire NZIS 1007 must be submitted, but the X-Ray's themselves are not required unless an abnormality is found. The blood results from the lab itself are also not required, just the NZIS 1007 section relating to the blood test findings.

Thanks a lot for prompt reply. How did you make your ITA application payment? I mean to ask whether details about our credit card is enough or not. If not, what is the procedure to make a payment with credit card? In addition, do we need to include Prepaid self addressed courier pack with our ITA application for returning our original documents? Good luck for your residence visa soon.

Yes, credit card details with number, expiration date etc should be enough. There is a place to fill in the information at that back of the ITA I thought.
I don't know about prepaid courier pack, I think they either send it back in a courier pack regardless or you can pay them later.

Also you can add additional pages, just refer to them on the amendments page.

It was my understanding that the x-rays only had to be submitted if the doctor said they had found anything worrying. We were told by the doctor's 'no problems'. When I picked the medicals up I was told that we had to submit it to NZIS unopened. So I took straight from the doctor to NZ House (in London) by hand, together with our ITA. The NZIS employee who took our application looked through the ITA, then through the medical forms and said 'oh, broken ribs', then looked at my SO's x-ray and said 'we'll keep this'.

I was very surprised because (a) I thought they were looking for things like TB on the x-ray rather than broken ribs and (b) the ribs are healed and when he broke them he also broke his shoulder and collar bone but that didn't get a mention.

If I'd been sending the ITA I wouldn't have sent the x-rays.

Hi Janbel -

I think your credit card questions has been answered, correct?

As far as the prepaid postage to return our documents via the "secure" method, I think this is down to those applications originating from the UK only. I am in the U.S. Our postal service would have no idea how much it would cost to send everything back prepaid via a UK carrier. And even if it did, it would not be able to sell us the postage. This confused me as well, so I went to the forums. Apparently, if you specify on you application payment form and/or migrant levy payment form that you want your documents sent back via DHL, or another well know international courier, using the credit card info provided, they will do so. That's what I did, but I only sent it in a few days back. We'll see what happens.
