Quite honest, I hardly seen anyone here got refused actually. (correct me if I'm wrong)

I've only known one or 2 people that its happened to - one personally and one through someone else telling me. Providing you've got the points you need when you submit ITA it seems to be just a matter of time...

I'm afraid I don't know the answer to your question, but I would love to know if anyone can give reasons for anyone being refused. Once your ITA's in, you start thinking all sorts...
