I asked the police to send me a form to request a police certificate for the purposes of moving to NZ. They sent me a form DP1:

DATA PROTECTION ACT: HOW TO APPLY FOR ACCESS TO INFORMATION HELD ON POLICE COMPUTERS

I'm not sure if this is the correct form, because on the first page it states,

"Evidence of Character: Exercising your rights under the subject access provisions of the Data Protection Act (1998) will NOT provide you with a "certificate of good conduct" or "Police clearance certificate".

What forms have other people filled in?

This is the right form.

All you need is the police report which will come back saying that you don't have a criminal record (unless you do of course :oops: ).

I found it abit misleading too. I actually took mine to the county headquarters to hand deliver and they assured me it was the right form.

Nothing is in plain English is it ........ I think they panic us like this to test our character in the hope that some will stumble along the way and not actually make it to immigration.

The Canadian immigration info is much clearer, it states:

UNITED
KINGDOM
Persons who have resided in the United Kingdom can obtain information held on police
computers under the Data Protection act. You may write to the following address requesting
your Prosecution/Conviction History under Section 21 of the Data Protection Act, 1984. Do not
request a police certificate.
Metropolitan Police
Subject Access Office
10 Broadway
London SW1H 0BG
United Kingdom

I'm beginning to think it's all just an initiative test and I'm failing.
