If you go into your local police station (if it is of reasonable size!) they often have the forms there, if not, ring them and they will arrange for some to be sent to you. That's how we did it, probably other routes...
I believe that the Met have an online system... I am sure someone on here knows of it...

For the online form......

Just print it off and send it.
They`ll send the reply to you whereever you are.

Remember that police checks can take up to 40 days to get back to you. Mine came back in just over 2 weeks but I have heard of others that have taken 6 weeks or more.
I don't know where you live but any documents that you can hand deliver will help to speed up your process by an all important day or two.
Good luck

I only found this out by accident and after I'd applied for my police certificate using the usual methods :wah
You can obtain a police certificate "on the day" if you arrange an appointment at a major police station - you'd need to check with your County Police Headquarters where exactly they do this - I was told Guildford in Surrey did this (you have to arrange it in advance - you can't just turn up - and they only do it between Monday - Friday office hours) but its useful to know if you are desperate. It also does cost more to do this).
My South African friend told me this as he has had to get one from the UK as he's lived here a while but we asked when we were in the police station and they confirmed it.
